I am a past client of this provider

Sawyer House is part of a corporation called: Assisted Living Concepts with a corporate office in WI. It is a nice building with nice rooms, but very understaffed. After 5pm, they only have one universal (does multiple tasks). They expect this one person to: fix dinner, help residents who need it, serve dinner, do dishes, wash table linens and re-set table, and clean rooms and give showers to anyone on the schedule, plus anything else that comes up. My Mom escaped twice because they didn't have staff they need. They have an alarm, but they can't check it if they are with another resident. They also don't provide toilet paper (only place I've ever known that doesn't)(basic need, isn't it?). They didn't tell me this until my Mom had been there 1.5 weeks. She'd had to use the one in the lobby. Then after they told me, I stocked it with a huge package, but they never replaced the empty roll when it ran out. It was always empty when I went, and also when others visited.
Her toilet was plugged up several times, and one time it was going into the weekend, and it took 3 or 4 days to have someone out to unplug it, so she had to go to the lobby to use the bathroom all that time (which she did most of the time because she didn't have toilet paper). She had dementia and didn't know how to get and refill it, but knew how to use it. I also paid for them to give her pills 5 times a day, but only one care giver (pm shift) ever came when I was there to give them. So she never had the pills she needed in the morning, and she went downhill until I took her to the hospital and she never returned. She was stable when I took her to this facility, but didn't stay that way.
The monthly bill was never correct, and now that she has passed away, they are trying to collect money for rent and care for months after she moved out and charge for things she never had. I gave notice 2 days after she left, and had her room cleared out in under a week) No one seems to have the same information, and I've had to retain a lawyer. They owe me a refund, and they are threatening to send me to collections. I sat through dinner for almost 3 weeks after she'd escaped and needed supervision (extra $13.??/hr round the clock), and saw that dinner most nights consisted of a cold sandwich (turkey ham or turkey on cheap bread), cream of something soup and ice cream. The weekly menu looks good, and they post the daily menus on a board daily, but what they served was not what was listed, so if you consider this, drop in at meal time unannounced and see if what they serve is what is on their board. I can't speak for the earlier meals since I wasn't there very often in the am. Her friend is a retired care-giver and has seen many many facilities from home health care to nursing homes, and has said she has never seen a place with such a lack of care and so poorly run. This didn't apply to me, but this place does not accept medicaid and does not provide transportation. They will call some kind of ride service and you pay for it.
They promised an activities director, but didn't have one until after my Mom's second escape (2 months after move in).
Beware of this place. They make promises they can't or won't keep. If for any reason you are considering it, please make every request in writing, and require them to produce everything they tell you in writing. Make sure you read and get a copy of the entire contract as you will need it if they make threats. I regret terribly that I ever took my Mom there. I would rate it less than one star if that was an option.
