I am a friend or relative of a current/past resident

Please, if you love your family member do not place them here. Staff is absent, front desk is empty, there is no communication, patients are left in bed ALL day, therapies are shortened and rushed. It is a sad place with little to no resources and/or support. Admin is just as bad and uninformed. My mother regressed here and caught a cold sore. Be mindful and cautious.

A nightmare! Try calling the main number, half the times you don't get the operator. When you try choosing a specific dept, no answer. You leave a message. No one calls back. You text a staff member, eternal wait for someone to get back to you. You go through each floor and there is a reception desk with NO ONE there. You ask a staff member that you need to speak to a nurse/ doctor, you'll wait forever. Very limited nurse/doctor service. so if you're there to do nothing but sleep,,fine. If you're there for any type of rehabilitation/therapy or anything other than staying in your bed and having someone bring you your meals...forget it! I told a nurse that my dad was feeling a sensation where he had the catheter, she said she would call the urologist. Never heard from her again, two weeks later he has an infection. When I finally spoke with a doctor, he was checking the records and came across that my father's potassium level was high. He then was going to start him on something. Made me wonder if I hadn't ask for an update, he would've never done anything to try to lower it. While I am writing this, I'm still waiting for a call back from the doctor, from the social worker and the therapist. Some messages were left a week ago...

Communication with The Plaza Rehab and Nursing Center is absolutely atrocious. It was really hard to get a hold of my father, and visitation is Monday to Friday 9:00 to 5:00 or whenever their rec person is available. We haven't gone in too far because of COVID, but from what I've seen the layout is nice and it's very well-kept. I wouldn't say that there's any looks of distress or anything. I've observed activities, but there's nothing that he's been invited to; that's the problem. During one of our visits, they were having activity in the next room where they were playing music. That was causing a problem for us to hear him because he's already behind plastic, and then we couldn't hear him because of the music that's going on in the back. So that was aggravating. He has insurance that's been covering it so far. I think they should put a lot more effort into showing the families that they care about our peace of mind, that he's being well-taken care of there. I don't think it's terrible. I just think that there are many things that could've been done better. Quite often I would call, and it just keeps ringing and ringing. I'd have to call several times a day before I hear from somebody. If each floor has its own crew, there should be periodic calls to the family, especially in this case where there aren't any phones in his room. When they can do the video chat, it's usually during the middle of the day when I'm at work.

I visited this facility

The Plaza Rehab and Nursing Center is a bigger facility than where my mother is at now, but you probably only have one CNA for the whole floor. There's like one nurse with 20 patients. It's not fair to the individuals, and it's not fair to the nurse. They won't be able to properly attend to all those individuals because they have to go through all those patients within those hours that they have. They have to wash them and feed them; it's a lot. She's got her own room, but with the other rooms I believe they have 4 people to a room. My mom was nearly there for 2 months and never took a shower. The therapy that she got there wasn't good at all. Where she's at now, she's able to move her arms after just 2 weeks. She was in The Plaza for two months and wasn't able to do anything. I can't blame the staff, the staff was good. They would do what they had to do, but it was overwhelming for them. The food was okay. It was hard for me to get her out of there. It took a whole month to get the paperwork from one floor to another. It was like they were holding her hostage. People wouldn't answer the phone.

My mom was admitted to this facility with a Covid case and my sister was not informed. When my sister took her no one told her there was a covid case. Unfortunately, due to covid we were not able to visit our mom while her stay there. We didn't have a chance to see the room where my mom was in and we were never given any information what activities they offered. The communication in this facility with the staff is horrible. The social workers do not return messages. The administration is very vague with any information you need. No one in this facility are on the same page. My mom was there a month and suffered a fall which we were not given details on how she fell and what steps they did to make sure she was not hurt. Due to the covid cases we weren't able to visit our mother is person and the so called window was impersonal. This facility had covid cases every 14 days. Something is definitely wrong in this place. We did have the opportunity to talk with our mother via face time but as we all know it's not the same as being able to give hugs and kisses or say I love you in person. My mom passed away in this facility and we didn't get a chance to spend more time with her. Our hearts are broken because our dear mom is no longer here. We do Thank God she did not die from Covid. I do not recommend this facility to any one. I cannot rate this facility on anything because there is nothing to rate it on especially with the way we were treated. Please make sure you have other options before you sent your loved one to this place. It doesn't have the same reputation when it was called the Jewish Home. As we all know if you have money you get treated well and when you don't no one really cares what happens.

I visited Jewish Home Lifecare, and it's a little bit on the pricey side. I met with the staff, and they seemed OK.

I visited this facility

Jewish Home Lifecare – Bronx is right close to everything for your everyday life. You could take the bus or go out; you don’t need be stuck in the nursing home. It’s very nice and very clean. It has a beautiful dining area. It has like a park where you could take the person out. Their doors have designs; it's very tricky. People may get confused and try to go out. They have a button that they push if they come out or take the elevator; they cannot go to the stairs. I really like them, they raise my spirit up, but at the end of the day, they say, "We don't have a place," or "We don't have any beds." However, it's a very small place, which I felt shady about.

I visited this facility

My father was 81 year old and was suffering from multiple illnesses. He spent the last years of his life in and out of the hospitals, mainly due to complications with high blood pressure. During the last emergency call to 911, my father was taken to a city hospital where surgery was performed on him but his health took a turn for the worst. After doctors decided that nothing further could be done for my father, my family took him to Jewish Home Life Care, where he received excellent medical care until he expired. The staff at Jewish Home Life Care were always very professional and treated my father with dignity and respect. The facilities always seemed clean and inviting. My family and I have nothing but praise and gratitude toward Jewish Home Life Care.
