I am a friend or relative of a current/past resident

the place is very nice(to look at ) thats the only good thing about it. my mom was there for 6 days and then i brouht her home. she had no showers in that time, her food was always cold,she couldnt get coffee they would say ill get you some and never come back.she was even told they were out of coffee.her room was never cleaned and her roommate had to really get on the staff to get the bathroom cleaned.she was admitted to one room and the next day was moved to a completly different wing

I visited this facility

We toured Towne Center Community Campus. It's very upscale, very well maintained, and very clean. All the ladies that work there that we talked to were very friendly and knowledgeable of what they were doing. They have a happy hour at 3:00 every day, they have a theater room, and they have bingo, too. They have crafts there, and they mentioned that they do field trips. We looked at a one-bedroom apartment and it was very nice. We looked at a studio apartment, and it was also very nice. They seemed to be roomy. They have housekeeping and things like that for it. They're in very good condition. The rooms looked like they were brand new. If my mother-in-law was eligible for assisted living, she would have been there.

The food was terrible, most of the aides were not helpful because they were always on their cell phones, always waited at least 20 minutes for my call button to be answered, one staff nurse and one office employee kept going through my personal belongings without my permission, therapists didn't care what time they came for you sometimes interrupting either breakfast or lunch. Need I go on? Never saw a cleaning person and had to ask for someone to wipe my tray off.

I visited this facility

My mom will be moving into the Towne Center Community Campus. It was the most expensive of all the places we looked at, and we chose it because it had the most options. Towne Center has assisted living, but they also have a nursing home. So years down the line, if my mom needs to move into a nursing home, she can stay all within the same development. The convenience of having multiple options in one facility as well as Towne Center will take Medicaid when my mom runs out of money. So in three years, when my mom's money has worn out, I won't be having to move her. She can still stay there, and Medicaid will pay for it. It's a one-bedroom with an efficiency of an apartment, with a kitchen, a microwave, a refrigerator, and a small sink, which is great. It's got a full library, and my mom loves to read. It has a game room. It's got happy hour every day. It has a nice little outside area where she could go outside to read her book. The one staff member that we interacted with was amazing.

When we looked at the apartments and were given the tour, one of the selling point was that there would be transportation to doctor's appointments when scheduled. This has not been the case for the last several years. There were issues getting my relative to see her dying husband in the same facility. She has been told she can get a ride to her appointment but not home. Luckily, we have been able to accommodate her appointments so far. How do you tell a 96 year old woman you can take her to her appt. But can't get her home? They need a different system or not to say thete is transportation available

A relative has been in the assisted living part for a few months. There are hardly any activities on the weekends for the residents. The front desk /main phone is unstaffed on weekends and after 5(?). Seems like they’re short staffed. Most of the staff they do have seem nice, though. The rooms are adequate.

My mother-in-law moved into the assisted living of Towne Center Community Campus. It's good, and she seems to be enjoying it. They seem to have activities every day, so she tends to like that, and she's doing good. The rooms are great, they're big, and spacious. The staff is cordial, and we do not have any issues with the staff. She said the food is pretty average.

My mother is in the last stages of Alzheimer's, and she's in Towne Center, which is a good place. The staff is pretty good, they're caring, and they do what they're supposed to do. The place is very well-designed, clean, and modern. My mother's room is well-kept.

My 23 year old daughter stayed in the facility for 11 days and received what I consider superior care! The staff, PT's, Nurses and Dr.'s all went above and beyond in their treatment of her and they kept us informed every step of the way. I would happily recommend Town Center to anyone in need of a skilled nursing facility.

Having worked in the health care profession and having been a patient there myself, I have to give this facility a 1 star review. On the positive side OT and PT are very good, but unfortunately the good doesn't outweigh the bad. As mentioned in other reviews the STNA's were constantly understaffed. At times there was a 1 to 32 staff/patient ratio and it was not uncommon to wait as long as 2 hours for staff to answer the call button. Considering the type of patient needs there, this was a definite life safety risk for patients, and during my stay patients fell in their rooms without being discovered for a long period of time. As far as nursing went there were some very good and dedicated nurses, but there were others who frankly seemed incompetent. As with STNA's, nursing also had to deal with under staffing. At meals, especially breakfast, hot meals were served on ice cold plates that quickly lowered the serving temperature. There seem to be no attempt whatsoever to serve meals at the appropriate temperature as required by nursing home regulations. And yet again under staffing was a problem in this area as well. I put the responsibility for these deficiencies squarely on the shoulders of the administration who apparently thinks that a fancy, elaborate building decor makes up for inadequate and substandard care. It does not. I cannot recommend Main Street Care Center to anyone.

I don't know the quality of their physical rehab but their nursing is sub-standard.. The nursing staff is under maned and overworked. I met only one nurse that was good. The management is disorganized and could not answer basic questions on the operation of the organization. In a ten day period the laundry lost and could not locate multiple items of clothing that was given to them for washing. I urge you to look elsewhere for a facility that functions. This one does not.

Let me start by saying our mother died within 3 months of being placed in this facility. Lack of adequate staffing was the primary cause. There were other factors as well. The place has only one or two people on duty holidays, weekends and evenings. For the most part the residents are left alone and very far from the staff. It's beautiful, it's clean, Main Street Campus came highly recommended as a 5-star facility and we were thrilled when they had a bed available. But it is the worst facility you can put a loved one in who needs skilled care or has any medical issues that need attention or close observation. Residents are essentially warehoused there in lovely surroundings and although we visited our mom often, she said she was the loneliest she has ever been. She had been in nursing home care for nearly 10 years and this is the worse experience we've had in terms of lack of engagement and support of quality of life, total lack of coordination of care and lack of adequate staffing. The few people that are there care but do not have the time or support needed to do a quality job, therefore mistakes are made and people are alone neglected. The TV is used as a baby sitter. Placing our mom at Main Street was the worst decision we have ever made in caring for our mother and she paid dearly for it. Don't be fooled by appearances or ratings. The rooms seemed a lot nicer at Towne Center, and it’s fairly-new. It’s big, but has a homey feel. I looked at some of the independent-living apartments, and it was really nice. The people seemed fine.

While this facility is clean, attractive and looks well planned out, it is woefully understaffed. It is not the fault of the nurses who we believe are doing their best, but top management and the protocols set up for attending (or lack of) physicians. We had our Mom moved here after major surgery thinking she would receive the best care and instead almost died twice. They coerced her into doing P.T. telling us Medicare would kick her out if she didn't participate; even though she was gray, with blue lips and sweating profusely for a week. She had severe pneumonia. If not for family members calling 911 two different occasions she would have died. She was also diagnosed with C. Diff diarrhea and after 6 months I finally looked at her chart (my stupid fault) and they also had her on laxatives the entire time. Her Primary Care physician had her so loaded up on drugs that she didn't even know where she was. I would like to name this local female physician, but will not; however she only saw her once a month. Please do not send an ill loved-one here.

My grandfather was placed at main street for rehab after a broken hip. The facility it self is beautiful but that is about it. They were always short staffed, one nurse and one stna for 20+ patients. Management didn't seem to care when complaints arised due to short staffing and improper care. I would not recommend this place, not because the nurses or stnas were incompetent, but because the company is for profit and does not care for their staff or the patients they take care of.

We looked at Towne Center. The experience was OK. The facility was nice and small. The rooms seemed smaller than the other places. They seemed to have plenty of activities.

I am/was a resident of this facility

I broke both my hips, and I was in Town Center for rehab for 12 days. It was gorgeous. However, they don't have enough help. I was in Orthopedics where people had broken hips, knee surgeries, etc. They had one RN and one aide in the evening, and I don't think they had much more than that during the daytime. At night, they had a man aide like if you needed help with the bathroom. I had a nice room, and everything was beautiful. You had to go down for breakfast, lunch, and dinner, and it was quite a hike, but walking is good, so that was fine. The meals were OK. They had bingo, cards, and day trips. However, one RN and one aide for a whole floor is not right. I don't know what they do on the other floors.

We had heard nothing but accolades for this place, but my mom went in for simple hip replacement rehab and it was the worst experience we've ever had. This company is out to make a buck, nobody cares about rehab patients. They are put in with nursing home patients and left to care for themselves. If you want ANYTHING, even a pitcher of water you must ask for it, then you may never see it. Physical therapy on the weekend was practically non existent. Once the weekday arrived, my mom got gastroenteritis, along with the two patients in the rooms next to her, with whom she had never interacted. She was too ill to even participate in therapy which was the reason she was there. The only positive was Dr.Voytas and his nurse, who were the only people who were dependable and gave any care. We pulled her out early and plan to spread the word to everyone we know.

I visited this facility

Towne Center Community Campus was neat and clean. It didn't look like a nursing home, and the staff was very nice. I would definitely recommend them. This would be number one in my list. I will tell my kids to send me here if I had to go to a nursing home.

Towne Center has different levels of care. You can move from one level to another if the need arise. The staff is fine. They're good. I have a two-bedroom apartment and it is fine.

I am a friend or relative of a resident

When I could no longer keep my mom safe at home, my sisters and I researched and visited 6 area nursing homes and prayed for a spot at Main Street, which is part of the Sprenger Family Facilities at Towne Center. Mom lived at Main Street for almost exactly two years, and it was the best thing I could have done for her. The nurses, aides, therapists, activities director and dining room and cleaning personnel all treated her like a loved member of the family. Mom was able to thrive there, far better than I had been able to care for her at home. There was constant interaction with staff and other residents. Any issues we migth have had were addressed almost instantly by the D.O.N. and the Administrator. When she was dying, the staff provided our family with food and as much quiet time with Mom as could be. I often promote Main Street to people in the community, and should I ever need rehab, I would absolutely suggest the PT department in this wonderful facility.

Quality of care was horrible. My parent ended up passing away. We were overbilled for prescription drugs though I received a call three hours my parent was removed to go to a hospital due to a serious medical care of which my parent died from that they had given my my parent no prescription drugs since her wonderful doctor never called in for EIGHT DAYS. So, what - we were billed 1,800 for an aspirin? THIS IS A FOR PROFIT CENTER AND PEOPLE SHOULD BE AWARE THAT THEY DO NOT CARE - THEY WILL CHECK YOU OUT WITHOUT GIVING YOU THE PROPER SKILLS AND THEIR SOCIAL WORKER STAFF IS THE WORST I HAVE EVER SEEN AND I WORK IN OHIO AT A NOT FOR PROFIT AND YOU CAN HONESTLY SEE THE DIFFERENCE. THEY JUST WANT TO MAKE A BUCK, SERVE THE WORST FOOD, AND TREAT MANY PATIENTS LIKE THEY ARE THE SCUM OF THE EARTH.
