Isabella Adult Day Health Care Program

We offer personalized health care, recreation and social activities for chronically ill adults who live in Upper Manhattan and the Bronx. Care is provided according to the assessed needs of the client.

You can schedule a day to visit our program at no cost to you. Spend some time with our caring staff and meet the members of the program. Stay for lunch and enjoy some activities. Transportation is available upon request.

Eligibility:

• Must be 18 or older
• Must be referred by a physician
• Must suffer from a chronic medical or mental health condition
• Medicaid, Managed Long Term Care or Private Pay is accepted for payment, assistance is provided to obtain Medicaid eligibility
