My mother was recently (3 weeks ago) admitted to a nursing home for physical therapy and strengthing after being in the hospital.  She has some dementia, with increasing confusion.  She is not safe to be alone, so that rules out going home, or living with a family member who works during the day.  My question is:  once she is physically stronger, how do we determine what type of care facility is best for her?  Does anyone have experience with someone going from a SNF to assisted living?  My mom does have LTC insurance, but I have to get the company to send me an explanation of her benefits (which may determine where she ends up).  Any info is greatly appreciated.  Oh, the most important part is that I am across the country from her, so I have to depend on a very few friends or my cousins to give me info on her condition.

How does a person decide on what type of facility (nursing home of assisted living facility) is best for their parent? My dad is 90 and was independant...drove himself to doctor's appointments, shopped, cooked, took care of his finances, etc. until 2 months ago. He passed out, woke up and was dizzy and couldn't get up from the floor. He went to the hospital for 5 days then went to rehab for 3 weeks to get his strength back. We then made plans for him to move into an assisted living facility. He was reluctant at first, but decided it was for the best. He went back home after rehab and was doing ok for a few weeks and passed out again. This time he was in the hospital for 10 days and became very weak and extremely depressed. We brought him to our home for a few days before he was to go into the assisted living facility. He was unable to get up out of bed to go to the bathroom and layed there crying for help. My husband tried to help him up and into the bathroom with the aid of a walker. Dad stumbled, fell into the door jam and caused a skin tear on his elbow. I drove him to the emergency room for repair. They bandaged his elbow and sent him home. He still couldn't walk or get up from the bed or even a chair. He then layed in bed and cried and kept saying he wated to die, please let him die. I called 911 and had him brought back to the hospital. Now we have to hope that he is able to build up some strength to be able to walk and assist himself in order to move into the assited living facility. However, I afraid that this situation may occur again and he would not be able to stay in the AFL. Do we move him from rehab to a nursing home? How do we know what is the right place for him?
