Setting goals for inpatient rehabilitation

I wanted to find out if anyone else has set goals for a loved one undergoing inpatient rehabilitation?

My Dad was transitioned from ICU to a nursing home 8/13 and with a brief stint back at ICU on 8/15, he returned to the nursing home for rehab. He is angry and wants to go home. My Mom & I have been talking with the social worker a great deal recently trying to decide what is going to be in the best interest of everyone. I put together a list of goals for my Dad and I shared with my Mom and she agreed with them. I shared the goals with the social worker since my Mom was going to meet with her today. The hope is that my Dad will understand that we love him and do want him to come home but that he needs to be able to do basic things such as step up 4 stairs to get into the house, carry his oxygen & not trip on the tubes, get to & use the toilet, walk steadily even if it means using an aid, sit up straight so he does not fall out of chairs and to assist his breathing, and perform basic self care such as brushing his teeth or shaving.

My Mom had a heart attack last year and had a stent installed to relieve a blocked artery. She's doing well but cannot be lifting or supporting someone who weighs 50+ pounds than she does. I'm not in a position where I can quit my job and leave my husband & son behind to move in with them as a permanent care taker.

I got long winded. :-) I'd like to hear if anyone been through something similar.
