I live out of state, my mother lives in a house that I wouldn't wish on my worst enemy, there is no water, no power, no functioning bathroom, finally we got the landlord to get her in another house but she refuses to take our help and move, she is an alcoholic with a pace maker, she has not gone to any of her appointments, her "friend" locked her in the house with no food or water and she ended up in the hospital, she checked herself out and is still living in filth. She uses the bathroom on herself and doesn't get clean she just takes the underware off and throws them in the corner or out in the yard. I Think we should take guardianship over her and put her in a facility

Wow. It sounds like your mom desperately needs some intervention. I'm definitely not an expert, but my advice to you would be to consult a family lawyer about what action you can take. Her living conditions sound, frankly, illegal. A "friend" locking her in the house (with no provisions) sounds criminal.
